Significance of the document:
Both sets of Generation-3 parents are named.
It appears from the scan that Samuel's number of times previously married was intentionally left blank, Anna has a clear, typed "0" there. This is a copy (from bad quality of witness signature, a carbon copy probably).
Minor discrepancies/variants:
Anna using the "C" spelling of her last name,
Anna's age is several years too young (suggesting perhaps she was never explicit about it with Samuel?)
Anna's mother's last name is spelled "Chick" instead of "Czech".
